My I ask where may I find the setup sheet mentioned in this tutorial? Thank you in advance!

Ahh bugger, I'm doing some more tinkering and testing, sorry about that guys I thought it was solved. I have it working again for me now in a test account so feel free to check again.

I want to check a few more things before I call it a fix this time though, so if it still doesn't work don't panic yet.

In the meantime I have also attached the sheets to this post.
