In the “concentric twisting” video we’re told to twist shielded pairs in the opposite direction they were laid to avoid forming a quad. Could someone explain why? Is it a noise issue or would it just create a problem with the next layer?

This is not because of a layering problem.
It is because if you twist them in the same direction the 2 pairs will become aligned with one another, forming this "quad". Meaning you will lose the ability of those pairs to cancel out the interference.
